Here's where you will find all the necessary information you will need for your first visit with Dr. Weeden.

The following forms should be completed prior to the first visit. Please bring the completed forms to the New Patient Coordinator at your first appointment:

• New Patient Questionnaire: This form includes contact and insurance information as well as your medical and dental history.
• Notice of Privacy Practices: This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.
• Acknowledgement of Privacy Practices: This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.
• Privacy Consent Form: This form is optional under the new patient privacy regulations recently issued by the United States Department of Health and Human Services. We have elected to use this form. Prior to commencing your orthodontic treatment, you should review, sign, and date this form.
